Yes I got the definite feeling while listening to this song that it was about their fans. It basically makes the statement that no matter what you do, someone is going to be #!@%*# off. You can't please everybody. DT's had couple of albums that I wasn't exaclty thrilled with, but I don't through a fit and send hate mail. I simply wait for the next album. Probably the biggest shocker for me was Systematic Chaos. I was like, "What? What is this ^$@#&^? But there a few good songs on it that helps me keep the faith.

This song is great. They really outdid themselves making this song
If you can't tell, this song is about the fans from #!@%*#. (no rhyme intended) These are the fans who are never satisfied, the ones who say "I so totally love your music!" and then get downright hysterical that their fave band member didn't write them back, he must be too good for me, he only cares about himself!
The exaggeration about him cutting himself open is rather gruesome, yet it works. Dream Theater try their best to cope with and please their downright obsessive fans. (not that all of them are that way)

A band is always on the road, spending insane amounts of time on the road, and those obsessive fanboys want more and more tours in their region, so they can see the almighty Jon Petrucci, James LaBrie, John Myung, and so on in person. The band probably has to keep up whole nights to choose the right songs. Cause if they choose the wrong one, a hurricane of hate mail comes in.
They give everything, and it's never enough.
